It has come to the notice of the Management that on [Date], you were involved in [describe the alleged act of misconduct]. This act, prima facie, amounts to a serious violation of the Company's code of conduct and service rules.

You are hereby called upon to show cause in writing, within [X] days of receipt of this notice, as to why disciplinary action should not be initiated against you for the said misconduct.

Should you fail to submit a satisfactory explanation within the stipulated time, the Management will be constrained to proceed with appropriate disciplinary action on the basis of the material available on record, without any further reference to you.
