Annexures I explained: this guide covers what it means, who it applies to, the step-by-step process, documents required, fees, due dates and penalties in India — so you can stay compliant with confidence and avoid costly mistakes.
The seven Annexures hold the lists that rules 54, 55 and 63 call "illustrative": what goes in a first aid box, an ambulance room, a mine first aid box, a mine ambulance room, a mine first aid station and a motor transport medical facilities room. Annexure VII is the list of all 27 forms.
Annexure I: first aid box (rule 54(ii)), 17 items. II: ambulance room (rule 55(ii)), 49 items. III: mine first aid box (rule 54(iii)). IV: mine ambulance room (rule 55(iii)). V: mine first aid station (rule 54(ix)(B)). VI: motor transport medical facilities room or dispensary (rule 63(ii)), 49 items. VII: the 27 forms. The lists are illustrative: the employer may add items.
Where the Annexures come from
Rule 2(1)(d) of the Occupational Safety, Health and Working Conditions (Central) Rules, 2026 says "Annexure" means a list annexed to the Rules, and rule 2(1)(b) says "FORM" means the forms annexed in Annexure VII. The rules that call for the lists are covered in our articles on rule 54 and rule 55. The Annexures themselves are in the English part of the Gazette text from the page headed "Annexure I".
The Central Rules apply where the Central Government is the appropriate Government. Where the State Government is the appropriate Government, the State's own OSH rules apply. For help preparing a first aid and medical facilities plan, see our labour law compliance service.

Annexure I: first aid box contents
Rule 54(ii) requires, in each shift, every first aid box or cupboard to be in the charge of a worker or supervisor holding a first aid certificate, at least one fully equipped box in every department or section, and more boxes if there are more than 200 employees in a department or section. The Annexure I list has 17 items:
- Small, medium and large sterilised dressing pads, in appropriate numbers
- Large sterilised burn dressing pads
- Packets of sterilised cotton
- Cetrimide solution (1% to 3%) or a suitable antiseptic solution (10% Povidone Iodine), in sufficient quantity
- A pair of scissors, tweezers, tourniquet and disposable gloves
- Adhesive tapes and adhesive bandage tape of different sizes
- Elastic crepe bandages of different sizes
- Sterilised eye pads
- Strips containing analgesic (Paracetamol 650 mg) and antacid, and ORS sachets
- Pain relief or analgesic spray
- Polythene wash bottles (normal saline or distilled water)
- Roller bandages of different sizes
- Suitable splints
- Resuscitation equipment such as an ambu bag
- Tablets such as aspirin and isosorbide dinitrate
- Anti-allergic medicine and eye drops
- One copy of a first aid leaflet which includes emergency contact numbers of nearby hospitals
Rule 54(i) also requires a notice, in English or Hindi and in the local language, with details of first aid facilities, the name and contact details of the person in charge, and the nearest hospitals. Rule 54(iv) requires at least thirty-three percent of workers to be trained in first aid, with refresher training every three years. Our rule 54 article covers these.
Annexure II: ambulance room equipment (49 items)
Rule 55(i) requires an ambulance room or dispensary where an establishment ordinarily employs more than 500 workers, and rule 55(ii) says it contains at least the equipment in Annexure II. The list runs from furniture and basic items (glazed sink with hot and cold water, table and chairs, refrigerator, examination table, kidney tray, thermometers, torch) through medicines (tetanus toxoid, antihistaminic, antispasmodic, antipyretic, antacid and antiemetic tablets; isosorbide dinitrate 5 mg; aspirin 75 mg with atorvastatin 20 mg and clopidogrel 75 mg; ORS; injections of atropine, adrenalin, hydrocortisone, furosemide and local anaesthetics) to instruments (syringes, suturing material, forceps, scissors, scalpel, stethoscope) and emergency equipment (oxygen cylinder or concentrator, blood pressure apparatus, intravenous fluids, cannulas of 18G, 20G and 22G, infusion sets and stand, blood glucose kit, pulse oximeter, suction unit, wheelchair, scoop stretcher, and resuscitation equipment such as an automated external defibrillator and ambu bag). Item 31 is biomedical waste bins as per statutory requirement, and item 49 is any other equipment the qualified medical practitioner recommends for the specific need.
Annexure VI repeats the same 49 items for motor transport medical facilities rooms, except that the final item refers to the Medical Officer instead of the qualified medical practitioner.
Annexures III, IV and V: mines
Annexure III (first aid box, mines) is organised by purpose: (i) wound care (sterile adhesive bandages, gauze pads and dressings, transparent film dressings, antiseptic wipes or solution, skin closure strips, disposable gloves, scissors with rounded tips); (ii) CPR and resuscitation (pocket mask or face shield with one-way valve, disposable bag-valve-mask device with PEEP valve); (iii) medications and ointments (antiseptic ointment, hydrocortisone cream, pain relievers, antihistamines, aspirin for suspected cardiac events, sugar sachets); (iv) bandages and dressings (elastic bandages and ice pack box, non-stick pads, burn care items including hydrogel dressings, sterile burn sheets and burn gel or spray); (v) equipment and tools (digital thermometer; suction apparatus, stiff neck collar, airway, manual breathing unit bag, finger probe pulse oximeter); and (vi) a flashlight with adjustable brightness.
Annexure IV (ambulance room, mines) has thirteen heads: (i) advanced medical equipment (portable oxygen cylinder with regulator, nebuliser, suction device, AED, blood pressure monitor, glucometer); (ii) sanitation and hygiene; (iii) patient transport, including plastic covers for amputated parts with ice packs; (iv) hygiene and wound care; (v) wound dressings; (vi) antiseptic solutions; (vii) patient comfort and warmth; (viii) splints and support; (ix) a pressure bandage; (x) non-contact infrared thermometers; (xi) emergency medications and equipment as prescribed by a qualified medical practitioner; (xii) anti-tetanus serum and other emergency medications; and (xiii) medical appliances, including one electrocardiograph machine.
Annexure V (first aid station, mines) is short: one stretcher with two blankets, inflatable splints, a patient examination table, and a first aid box with the Annexure I materials. Rule 54(ix)(B) says where stations go: above ground (top of every shaft or incline, every workshop, every screening plant and loading place, and any place with more than 50 persons at one time); in opencast workings (shovel or dragline loading points, rest shelters, pumping and refuelling stations, dump yard, any place with more than 50 persons); and below ground (bottom of every shaft, near every haulage drive end, in or at the entrance to every district or section). No station is required within 300 metres of another.
Points to note. Rule 54(ix)(B)(iv) to (vii) carry duties for the person in charge of a first aid station, a certificate from recognised institutes, an up-to-date station list at the mine office, and a first aid outfit carried by certified supervisors on duty. Annexure III item (iv) is printed with sub-items (c), (d), (e) and (f) under "Burn Care", as the numbering runs on from (a) and (b). Annexure III lists "Aspirin" and Annexure I "Asprin", a spelling slip only.
Annexure VII: the list of 27 forms
Annexure VII is the "List of Forms prescribed" under the Rules. As printed:
| Forms | Subject | Article |
|---|---|---|
| I to VII | Registration, cancellation, certificate, register, notices of commencement and cessation | Forms I to VII |
| VIII to X | Health examination (except mine employees), mine medical examination report, appellate medical board certificate | Forms VIII to X |
| XI to XII | Notice of accident or dangerous occurrence; notice of periods of work | Forms XI and XII |
| XIII to XVI | Employee register, attendance register-cum-muster roll, wage register, wage slip | Forms XIII to XVI |
| XVII to XX | Annual return, half-yearly return, register of accidents, register for leave with wages | Forms XVII to XX |
| XXI to XXIII | Contractor licence application, licence proforma, experience certificate | Forms XXI to XXIII |
| XXIV to XXVII | Audio-visual agreement, improvement notice, composition application and notice | Forms XXIV to XXVII |
The printed list has stray numbers ("1.", "2.", "3.") beside Forms III, V and XVII, which look like leftover footnote markers, and Form XVIII is listed as "Half Yearly Return (January to June / July to December) to be submitted by Contractor to the Deputy Chief Labour Commissioner (Central)". The list adds nothing beyond the forms themselves; use it as an index.
Practical examples
Example 1. A factory department with 250 employees checks Annexure I against its boxes and adds a second box, since rule 54(ii) calls for more boxes above 200 employees in a department or section.
Example 2. A mine plans first aid stations using rule 54(ix)(B) for locations and Annexure V for equipment, and keeps an up-to-date list at the mine office.
Compliance checklist
- Audit every first aid box against Annexure I (or III for mines) and record the date.
- Put a named, certified person in charge of each box and station.
- For ambulance rooms, check Annexure II or IV against the equipment actually present.
- Treat the lists as the minimum: the rules say the employer may add items.
- Use Annexure VII as the master index of forms.
