Under instructions of my client, [Sender name], your former/present employee, I serve you the following notice:

1. That my client served your establishment as [designation] from [date] to [date] under the terms of the appointment/employment.

2. That upon [resignation/relieving], a sum of Rs. [Amount] remains due to my client towards unpaid salary, gratuity, leave encashment and full-and-final settlement, which you have wilfully withheld.

3. That such non-payment is contrary to the terms of employment and the Payment of Wages Act, 1936 / the Payment of Gratuity Act, 1972, and my client is entitled to recover the same with interest.

4. You are hereby called upon to release Rs. [Amount] together with the relieving letter and Form 16 within 15 days of receipt hereof, failing which my client shall approach the Labour Authority / appropriate forum for recovery, at your risk as to costs and consequences.
