Forms LD explained: this guide covers what it means, who it applies to, the step-by-step process, documents required, fees, due dates and penalties in India — so you can stay compliant with confidence and avoid costly mistakes.
This second part of Schedule II covers Forms LD-19 to LD-35 and the agent forms LDA-1 to LDA-3. They fall into four groups: Registry forms for address, cancellation, advice, certificates, extension and review (LD-19 to LD-25 and LD-30 to LD-32); Board and High Court forms (LD-26 to LD-29 and LD-33 to LD-35); and the agent forms. The first part is in Forms LD-1 to LD-18.
This article reflects the Rules as amended up to G.S.R. 689(E) dated 14 September 2012 (the only amendment). Check sicldr.gov.in for later notifications and current forms and fees. Fees named are from Schedule I as notified in 2001; the Registry's forms page lists the same amounts (checked 3 October 2026). Choosing the right form for each step is part of IP portfolio management.
The Registry forms are LD-19 (address for service), LD-20 (cancel an entry), LD-21 (preliminary advice), LD-22 (certificate or certified copy), LD-23 (extension of time), LD-24 (review), LD-25 (duplicate certificate), LD-30 (extend opposition time), LD-31 (advertisement particulars) and LD-32 (authorise an agent). The Board forms are LD-26 to LD-29 and LD-33 to LD-34; LD-35 is the High Court appeal. No commencement notification was found for sections 32 to 53 (and 55), so the Board forms carry that line. LDA-1 to LDA-3 apply to agents.

Two points on the list. Form LD-35 is printed in Schedule II with its text but does not appear in the list of forms, which ends at LD-34 and then LDA-1. And the title of Form LDA-3 in the list speaks of the "register of trade marks agents", though the form itself is headed as an application for alteration of an entry in the register of layout design agents; treat it as the layout-design agents' form that rule 104 prescribes.
Notes on the Registry forms
Form LD-21: preliminary advice
The applicant requests the Registrar to advise whether the accompanying layout-design appears prima facie original so as to comply with section 7(1)(a), and gives an address in India for the advice. The title also speaks of "distinctiveness", while the body of the form asks only about originality. See Rules 21 to 23.
Form LD-22: certificate or certified copy
A request in the matter of a layout-design number for the Registrar's certificate to a stated effect, or a certified copy of a stated document, or a certificate of registration for use in obtaining registration in a named country. It gives an address in India for sending the result. See Rules 86 to 92.
Form LD-23: extension of time
The applicant identifies the matter, states his capacity, the period of extension and the purpose for which it is required, and sets out the grounds. The title limits it to a time that is not expressly provided in the Act or one for which the Rules already provide an extension.
Form LD-24: review
Filed in triplicate with a statement in triplicate; it asks the Registrar to review a decision of a stated date, the grounds being in the accompanying statement.
Form LD-25: duplicate certificate
A request by the registered proprietor for a duplicate or further copy of the certificate issued under section 13(2) for a stated layout-design number, with an address in India for sending it. (The form refers to rule 46(2); the list refers to rule 46(3).)
Form LD-32: authorisation of agent
To be stamped under the law for the time being in force. The principal authorises a named agent (a legal practitioner, a registered layout-design agent or a person in the principal's sole and regular employment) for a stated matter, asks that notices go to the agent, and revokes all previous authorisations in respect of the proceeding. See Rules 16 to 20.
The Board and High Court forms
No notification bringing sections 32 to 53 (and 55) into force was found in the India Code record or on the Registry's notifications page (checked 3 October 2026); check the current position before relying on them. The Act gives no other forum for appeals from the Registrar. The forms are:
- LD-26 appeal to the Board (verified by the appellant as true to personal knowledge and belief, with grounds based on legal advice tendered); LD-27 royalty application (benefit accrued from acts under section 18(1)(b)); LD-28 opposition to a royalty application; LD-29 review of permission to use; LD-33 cancellation of registration, assignment or transmission (with proof where the ground is non-originality or exploitation beyond two years); and LD-34 permission to use (stating whether sought for use by the Central or a State Government or an authorised person, with the efforts made and, for the grounds, non-commercial public purpose, emergency or urgency, or anti-competitive practice).
- LD-35 is the High Court appeal against a decision or order of the Board, addressed to the Registrar of the High Court. The rule prescribes it within three months or further time the High Court allows (rule 91).
The title of Form LD-26 on the form itself reads "under sub-section (3) of section 42" and in the list "under sub-section (1) of section 41"; the rule and the fee entry both refer to section 42(1). The rules for these forms are in Rules 69 to 74.
The agent forms
LDA-1 is filed in triplicate. It declares that the applicant is not subject to any of the disabilities in clauses (i) to (vi) of rule 95 and that the information is true to his knowledge and belief. It asks for name in full, residence, principal place of business, father's name, nationality, date and place of birth, occupation, qualifications, and whether ever removed from the register of agents and why. A certificate of character is enclosed, from an unrelated person who is a District Magistrate, a Chief Presidency Magistrate or the Chief Administrative Officer of the district, or another person the Registrar considers suitable; diplomas or attested copies go with it. LDA-2 restores a name removed under rule 101(1)(b). LDA-3 alters an entry. See Rules 93 to 99 and Rules 100 to 105.
A worked example
A foreign-owned start-up with no principal place of business in India files Form LD-19 to enter an address for service in India, and Form LD-32 to authorise a legal practitioner. It asks for preliminary advice on Form LD-21 before filing its application. After registration it needs proof of its registration for a filing in another country, so it asks on Form LD-22 for a certificate stating that purpose. If its application is refused, the Board appeal on Form LD-26 depends on a Board for which no commencement notification was found.
