Forms LD explained: this guide covers what it means, who it applies to, the step-by-step process, documents required, fees, due dates and penalties in India — so you can stay compliant with confidence and avoid costly mistakes.
Schedule II holds the forms the public files with the Registry. This first part covers Forms LD-1 to LD-18, from the application on Form LD-1 to the address-change request on Form LD-18. It gives the list (form, section of the Act, title) and then short notes on the forms most often used: LD-1, LD-2, LD-3, LD-6, LD-10, LD-11, LD-12 and LD-13. Rule 11 says the forms in Schedules II and III must be used where applicable.
This article reflects the Rules as amended up to G.S.R. 689(E) dated 14 September 2012 (the only amendment). Check sicldr.gov.in for later notifications and current forms. A filing set prepared under IP portfolio management should use the Registry's current version of each form. Fees named are from Schedule I as notified in 2001; the Registry's forms page lists the same amounts (checked 3 October 2026).
Form LD-1 is the application (section 8(1), rule 22), with a statement of particulars and three sets of drawings. LD-2 opposes, LD-3 answers, LD-4 gives notice to attend a hearing, LD-5 asks for grounds of decision, LD-6 corrects or amends, LD-8 to LD-11 deal with assignment, LD-12 rectifies the register, LD-13 to LD-16 deal with registered users and LD-17 and LD-18 change entries. The declarations in the forms say the particulars are true to the declarant's knowledge, information and belief.

Some forms carry notes that point to the Board: no notification bringing sections 32 to 53 (and 55) into force was found in the India Code record or on the Registry's notifications page (checked 3 October 2026); check the current position before relying on any Board route.
Notes on the main forms
Form LD-1: the application
The form is "to be filed in triplicate". The applicant states the title of the layout-design, the name or names, description and nationality (and for a body corporate or firm the country of incorporation or the partners), the address of the principal place of business in India if any, and the Registry office. A declaration says the particulars in the Statement of Particulars are true to the applicant's knowledge, information and belief. The form is signed by the applicant or an agent, and a note says drawings should preferably be on paper of 33 by 20 centimetres.
The Statement of Particulars asks for:
- the application number (filled in by the Registry);
- the title;
- a classification of the circuit that can be made with the layout-design by structure (for example Bipolar, MOS, Bi-MOS, Optical IC), technique (for example TTL, DTL, CMOS, NMOS, PMOS) and function (for example Logic, Memory, Linear, Microcomputer), each with "Other (specify)";
- a brief description;
- whether the layout-design has been commercially exploited, and if so where, in which country and on what date;
- documents enclosed: drawings or photographs (three sets), semiconductor integrated circuits (4 pieces, where an integrated circuit has been made using the layout-design), details of fees deposited and others; and
- the name and address of the agent.
The exploitation entry is the applicant's own statement, which feeds the register entry and the Journal advertisement under rules 30 and 44.
Forms LD-2 and LD-3: opposition and answer
LD-2 is filed in triplicate, refers to the application number and the Journal date, number and page of the advertisement, and states the grounds. Its notes say proof or details should be given if the opposition is on the ground that the layout-design is not original, and proof if it is on the ground of commercial exploitation for more than two years before the application. An address for service in India is required if the opponent has no place of business or residence in India. LD-3 is the counter-statement; it states the grounds on which the applicant or registered proprietor relies, admits allegations, and has a note on addressing it to the Registrar or, for cancellation, to the Board.
Form LD-6: correction
LD-6 is a short request. The signatory ticks whether he is applicant, opponent, registered proprietor or registered user, states what he asks to be corrected and says whether a copy has been served on the registered proprietor or user.
Forms LD-10 and LD-11: subsequent proprietor
LD-10 is a request by the new owner to be entered as proprietor of the layout-design as from a stated date, by virtue of a stated instrument (original and attested copy enclosed), and states whether the assignment was made otherwise than in connection with goodwill; if so, the Registrar's direction and advertisement copies are enclosed. LD-11 is the joint version signed by the proprietor and the transferee. Both end with a declaration that the facts are true to the declarants' knowledge, information and belief. See Rules 47 to 54.
Form LD-12: rectification
LD-12 is filed in duplicate with a statement of case in duplicate and copies for each registered user. It names the layout-design number and proprietor, says whether the entry is to be removed or rectified, states the grounds, records the appropriate office, and includes the statement "No action concerning the layout-design in question is pending in court." It is addressed to the Layout-Design Appellate Board or to the Registrar, with the Board line above.
Form LD-13: registered user
LD-13 is filed in triplicate with the agreement, the other rule 55(1) documents, the affidavit and two copies of each. The registered proprietor and the proposed user apply together; the form asks for the conditions and restrictions ("none" if there are none) and for the end date of the permitted use or a statement that it is without limit of period. Both sign. See Rules 55 to 58.
Drafting points in the list
- The title of Form LD-6 in the list reads "Request for clerical error or for amendment"; the form itself is headed as a request for correction of clerical error or for amendment.
- Form LD-19 in the list has no section number against it and is a request by a registered proprietor or user with no principal place of business in India to enter an address for service; it appears in the second part of this series.
A worked example
A fabless start-up files Form LD-1 in triplicate with its Statement of Particulars, classifying the circuit as CMOS technique with a logic function and stating that the layout-design has not been commercially exploited. It encloses three sets of drawings. A rival files Form LD-2 against the advertised application, arguing non-originality and giving details. The start-up answers on Form LD-3 admitting some allegations. After registration, the start-up sells the layout to a manufacturer; the two sign Form LD-11 jointly, enclosing the original deed and an attested copy.
